Ok, found it.
The module associated with the menu has a switch for "Check Menu", which was set to "On". Turning it off removes the check and therefore the fault.
But what's the fault about? And why is there a fault with the fault? What is it checking? The online help sayswhich begs the question, what's an "important menu item"?

Make sure you change "Check Menu" to No, otherwise your clients will get a notice to revert to the default menu.
